Chapter 5 Policy for Use of Library by Minors

Any minor child who wishes to use the facilities of the Greenblatt Library must have his/her parent or guardian sign a consent form approved by Georgia Health Sciences University Office of Legal Affairs. The consent form will be kept on file at the Greenblatt Library and must be updated on a yearly basis. The parent or guardian of any minor using the Greenblatt Library is responsible for any fees incurred by the minor and for any damage caused by the minor to the library or its collection.
Reason For Policy
The Greenblatt Library’s collection includes legitimate academic materials regarding anatomy and sexuality that some people may find offensive. Georgia Health Sciences University and its library staff do not monitor Internet usage, and Georgia Health Sciences University assumes no responsibility for any use of its computer workstations. Therefore, parents or guardians of minor children using the Greenblatt Library are required to sign a consent form giving their minor child permission to use the Greenblatt Library.
Entities Affected By This Policy
Any library patron who is under the age of 18, including children of Medical College of Georgia employees, is affected by this policy.
Who Should Read This Policy
Any parent or guardian of a minor child who consents for said minor to use the Greenblatt Library services and resources should read this policy. All library employees are required to read this policy.

The Greenblatt Library provides unfiltered Internet access to information. The library collection contains legitimate academic images of anatomy and sexuality that some people may find objectionable. Therefore, parents or guardians of minor children are advised of this information and are required to sign a consent form that has been approved by Georgia Health Sciences University Office of Legal Affairs for use of the library by a minor. The consent form is kept on file in the library and must be updated on a yearly basis.
Furthermore, it is against the law and Medical College of Georgia policy for anyone to use the Internet for any illegal purpose, such as the distribution of obscene materials to minors.
Georgia Health Sciences University reserves the right to revoke library privileges to those persons who fail to follow the rules of the Greenblatt Library, and to remove such persons if necessary. Parents or guardians will be held responsible for any fees imposed on a minor pursuant to the rules of the Greenblatt Library and for any damage to the library or its collection caused by said minor. The Library is not responsible for the safety or security of a minor beyond normal public safety standards.
The following procedures must be followed if a minor child wishes to use the facilities of Greenblatt Library:
- Library staff will supply a consent form to minor upon his/her request to use the library resources.
- Parent or guardian must sign and return to the Greenblatt Library a consent form for each minor child wishing to use the library.
- Signature will be verified by a library employee by making a telephone call to the parent or guardian.
- Form will be placed on file at the Greenblatt Library and retained for one year.
- Parent or guardian must sign a new form for every year of access to library services for each minor child.
The responsibilities each party has in connection with Academic Affairs Policy 8.05, Use of Library by a Minor, are:
